Please use the Reference Baseline Photo Guide listed below to match your new 2024 annual baseline photos with the correct Photo Number. The 2024 Baseline Photo Upload Form immediately follows the Reference Baseline Photos.

Please note that the multiple file upload process can take several minutes to complete once you click on the “Submit Baseline Photo Report” button. You should receive a message that the submission was successful displayed below the button once the server receives the data. This form has the capacity for 20 site photos, please ignore the extra fields if your site does not use them.
